Nutritionists during each nutrition counseling session seek to help the patient receive proper advice and adopt a total lifestyle change towards a healthy life (Kushner., et al, 2009). To achieve a fitness goal, the nutritionist is required to document some important details to properly track the progress of the patient in question. Josephine’s information ought to be properly filed. The important information includes; the current weight, body mass index, current diet plan, height, age and the progress of the client. The information documented will assist the nutritionist in coming up with a proper dietary plan and advising Josephine accordingly.
Provision of Josephine’s health information and nutritional Data to another health professional
The health and nutritional information should be confidential between the nutritionist and her. Revealing such information will lead to a breach of customer’s confidence in the whole process, and this is bound to affect Josephine’s self-esteem (Snetselaar & L.G, 2009). However, in the case where Josephine is hospitalized, the nutritionist may need to disclose some intricate details about Josephine to aid in proper medical control of whatever condition she may be suffering from; this is also important in the administration of different medicines.
In order for Josephine to continue to follow up, how do we address payment and reimbursement?
The best way for Josephine to make a proper follow up would be to base her payment monthly; this will prompt her to always make the visits to avoid losing her money. In the case where Josephine decides to discontinue the counseling session, and then she should be reimbursed on the balance left; this will create an element of trust between Josephine and the nutritionist which is particularly important for referrals and future references.
What motivational techniques would you use to motivate Josephine to continue making progress?
I would consistently take Josephine’s weight and draw up the information in a chart where she can be tracking her progress. I would also make her counseling sessions interesting by involving some training sessions. I would set up a weight limit and give her reasons for doing so, and this would make her really motivated to soldier on even when sometimes she feels like giving up.
I would schedule the follow up sessions on a fortnight basis as this would give her enough time to be making progress and also track her own progress. Delaying the counseling sessions for a long time will only derail her end goal while setting the meetings to soon will make her feel like she is not making any progress, and this is bound to de-motivate her.
Information I would you like to collect at the follow-up appointments
The subsequent information would have to be the kind of food she is now eating, her weight changes, what she feels about her new weight and also any challenges she could be experiencing on her fitness journey (Webster-Gandy., et al, 2006). There has to be a form of consistency for her to achieve the right desired results.
Ways In Which I Would Assess her previously set goals
I would take Josephine’s several measurements; this includes her weight, her body max index, her height and her fitness level. I would also evaluate how fit she is by giving her simple exercise tasks and observing her reaction. I would also engage Josephine in finding out if she really feels she has met her set objectives so that she can also feel like an active participant in this.
Obstacles That Could Impede Josephine’s Success At Achieving Her Goals and Ways I would Help Her
Josephine’s obstacles would include cravings and her mum’s food which seems to lack some important nutritional elements. Time would also be an obstacle most especially if her schedule is tight. I would offer her alternative solutions to her cravings and also encourage her to attend the counseling sessions with her mum; this will enable the mum to incorporate the lessons learnt in her cooking. I would also structure a well though time table that would fit into her schedule.
