Hello Kristina, I agree with you that the rationale for different programs in health care institutions is based on quality indicators. Depending on what the quality indicators show, a program can be launched. The bundled payment program that you are implementing in your organization is very appropriate because it borrows from the concepts of case management. It is my belief that managing the post-operative patients as out-patients gives them a relief financially, thereby allowing them to focus on the recuperation (Finkelman, 2011). This is very important in helping the patients achieve their health outcomes as desired by many health care institutions. The ‘Proven Health’ program is also important in achieving patient outcomes. Health education is very important, especially for patients who manage themselves at home.

Hello Tina, I was impressed with the program running in your organization. Firstly, the rationale for the program is grounded on policy matters of the governing agencies in health care. Secondly, your program is in response to prevailing quality lapses as evidenced by patient outcomes on the institution. Thirdly, the design of the program allows for a multidisciplinary cooperation, thereby helping improve the patient outcomes (Rubenfeld, & Scheffer, 2010). I also found that the manner in which the Diabetes Management Program affects the organization bottom line to be very elaborate. By working as a multidisciplinary team, the primary physicians are freed up, and therefore can aid in admitting more patients to the hospital. I agree that this program not only assists diabetic patients to achieve their health goals, but also contributes to the bottom line of the organization.

Hello Michelle, the continuous improvement program running in your organization brings together many professionals in health care under one common goal, ensuring that the patients receive care of the highest quality. I agree that this multidisciplinary approach enhances communications in patient care, thereby improving patient outcomes. It is also important that the program does not just limit itself to checking for consistency in the delivery of care, but also assists the patients with issues of reimbursement (Feldman & Alexander, 2012). This is because this represents the financial element that is important in delivery of care.

Hello Cindy, your discussion on the importance of continuity of care highlights its importance in one of the most important stages in child development. Premature infants carry an increased risk for health complications not only during that stage, but also later on in life. As such, I agree with your findings that parents should prepare themselves by seeking out other providers, preferably pediatricians. I agree that this is important in this instance to ensure that after the infant is discharged, it can still be cared for, be it by a new provider. This ensures a continuity of care (Hunt, 2009).

Hello Wanda, I agree with your take patient perspective issues are important in healthcare. One of the core nursing competencies learnt during the previous classes is that health care should be patient centered. While there are other approaches to achieving this, it is important that you highlight the need for care providers to actively seek any issues emanating from the patients regarding the delivery of care. The example you highlighted in your discussion highlights something that all doctor’s offices should consider. Administering surveys in order to learn patient perspectives enhances the doctor’s ability to offer care that is patient centered.

Hello Julie, I agree with you that timeliness of care delivery is a priority in health care today. I also agree that timeliness of care delivery is and should be a component for endeavors in quality improvement in health care institutions. Your discussion highlights the importance of timeliness, not just because it can be life saving in emergency situations, but also because patients want to be served with minimal delays (Huber, 2013).
