Incident reports vary according to the nature of the incident. A good report should include the following:
Fact finding- Research and find all the necessary information regarding the case.
Nature of the incident- Give a detailed description of the kind of incident and its extent.
Observations- Note down what you experienced before, during and after the emergency.
Actions- Give specific details on whatever actions were taken. Mention particulars e.g. person who performed the action, equipment used, etc.

Circumstances- Give the setting of the incident, focusing on anything out of the ordinary

Damages- Record any damages, describing the kind of damage and estimating the magnitude.
Cause- Where possible, determine what occasioned the incident.
Witness statements- Take statements from any witnesses if they are available.
Disposition- Describe what the situation was and the status of the victims at the time of exit.

Introduction
This memo is in response to your request for my progress report in my Business Communication course. In January 2014, your company, Acme Products Limited, sponsored my studies in this course with a promise to hire me during summer, if I do well in the course. The course is almost coming to a completion, for this reason, I am making this progress report to update you on what I have studied and how I have fared thus far. The memo will detail the kind of work covered, with comments on my performance and challenges during this course.

Progress

So far, the Business Communication class has proved to be very resourceful. We have covered many topics on communication in the market place. We have also handled several essays in different formats that have made me familiar with the various kinds of business writing. The different kinds of work that we have done include memos, business letters, business emails, and several different kinds of ways of relaying information. The course has also covered the writing process where we have learned planning, research, formatting messages, business language and proof reading and editing. The lessons have been valuable and I feel more informed and ready to take responsibilities that involve business communication.

Achievements

I into this class with raw writing skills. These have been polished throughout my coursework. At first the writing was challenging and it took me time to grasp the business tone well. I also had a problem mastering the various formats of different kinds of writing that are used in business communication. My initial essays required a lot of editing and often times I had to re-do the work, especially because my professor desires perfection.
Overtime, I have nearly perfected my business writing skills, improving my scores in my essays and receiving positive comments from my lecturer. My continuous assessment tests have been above average with some very good scores in some tests. My aggregate grade on the all the tests that we have done so far is A-, which is a great achievement.
I still have to complete my final exams. I am working hard on revising the class notes and doing extra reading using the coursework material so as to master the content of the course. I am confident that I will improve my grade to an A once I take the final exam. I have extremely enjoyed my studies in this course and will be great in my communication.

Conclusion

I have completed all the required coursework and final writing project. I am yet to do my final written exam for this course. My grades in the continuous assessment tests have been good. I feel I have achieved the objective as I have done well in my work and therefore qualify to be considered for a position with the company.
