The most important challenge that patients and their families may encounter in long-term care system is when there are changes in the socio-economic structure of a given location as a result of families and people moving from poorer to richer countries thereby adding to the demand for healthcare (Raphael, 2003). Many countries around the world are actually experiencing crisis in terms of the provision of heath care plans to its constituents because they could not afford to meet the growing needs of the populace. The complex configuration of long-term care may also suffer because of workforce shortages (WHO, 2003). As people in a certain city, state, or country continue to increase in number, the patient to nurse ratio or the patient to physician ration may deteriorate, causing strain to the quality of healthcare (Fall et al., 2012).
One important difference between a long term care and an acute care is the nature of treatment that is afforded to the patient. An acute care is medical care that aims to treat or medicate an acute condition such as a heart attack or anything that needs urgent attention. Long term care, on the other hand, supports individuals or patients that encounter hampering or disorder in their physical or mental conditions that may affect their mental and physical functioning. Such is facilitated by skilled, therapeutic workers.
I think the most important challenge that faces homeless patients with chronic illnesses is patient outcome. In fact, homeless patients receive tremendous amount of medical services; nevertheless, they are still registering a very low health outcome. Chronic illnesses such as congestive heart failure, cardiac arrhythmias, thromboembolic diseases, and HIV need proper abode to aid in the rehabilitative process included in their medication (Sadowski et al., 2009) . But because homeless people do not get the chance for an abode conducive for healing, they end up having negative health outcomes.
