Chronic diseases comprise of ailments that do not have a cure but through care and following the required medication, a person can control it (Staff, 2006). Chronic diseases do not choose the recipient; it occurs to anyone regardless of the demographics. Of late, people die in large numbers due to diverse chronic diseases. Most of them did not have an idea that they had it, some learned of it when it was too late. The few that survive have no option but to live with the condition for the rest of their lives. Most of the people later suffer depression due to the thought that rock their mind. Some of them give up on it because of the pain and cost. Most chronic diseases are expensive to maintain; from medication to hospital visits. Families of the affected deteriorate in terms of finances since most of their resources channel toward helping their sick members to fight the disease. Diabetes is one such chronic disease that affects many people and can become a burden to families and the society at large.
Recently, there is a high escalation of diabetic patients. People commonly refer to the disease as a rich man disease; mainly because it revolves around eating too much sugar, a necessity the poor man cannot afford (Staff, 2006). However, one can get diabetes if they do not have enough sugar in their body. The rising cases occur from the poor diet people nowadays have. The young people do not have a balanced diet because most of them prefer eating junk food for the day and go out to the nearest restaurant for their dinner. The salt and sugar content of such products combined causes harm to the body resulting to other ailments in the body. As stated earlier, most of the young people would not think they suffer from the condition because they rarely are diagnosed. Apparently, those that know they have it suffer from depression and lack a proper way to channel their depression.
Creation of a chronic care model will assist the individual and the community at large to battle and prevent such diseases from rising (Staff, 2006). The chronic care model works as an organization that main goal is to improve the care of chronic diseases, an individual and the community at large. The model requires everyone to work together to bring awareness about the disease. The researchers have to come up with better ways to fight the disease or probably a cure. Trained personnel like doctors and nurses have the task to update their knowledge concerning the chronic disease. New medication comes up daily, and the patient needs to use the recently dated medication to fight the disease. As the disease continues, it grows and becomes immune to the medication. That is why the medicine should improve by the day. There is a possibility for a cure to arise with a good care model.
Advanced practice nurses will come in handy in the care of diabetic patients especially the chronic cases. Their level of education guarantees them of the position they currently hold (Morgan, 2009). For one to qualify as an advanced practice nurse, one must undergo deep education concerning the particular disease. The education covers the background and the current situation. From such information, the medic can help in taking care of the patient and the research of the condition. Their major role in such a situation is to advise the patients on the best steps to take in making sure they keep the disease at bay. Apart from that, they will gather information from their patients and send it to research centers to help them in coming up with better medication.
