In this aspect, the questions that should be included to ease the completion of the survey should focus on the operations of the improvement initiatives for instance: What are the challenges facing the new improvement initiative? Are employees/ subordinate staff satisfied with the improvement initiatives?
How to Distribute the Survey among Respondents
Since the survey is focused on an evaluation of an improvement initiative, the persons who complete the survey questions should be people who are directly affected by the improvement initiatives. In this regard, the survey should be distributed among randomly selected respondents comprised of people who interact directly with the improvement initiatives. These respondents may include: Workers, managers and clients who are affected by end products.
Appropriate Time Frame for Completion of the Survey
The time frame for completion of the survey would entirely depend on the time allocated for the entire evaluation project. However, it would be appropriate to complete the survey at the same time, meaning that they survey can be completed within a day in case the respondents are workers within the company. Similarly, in the case that external respondents are required to complete the survey, a specific day should be allocated for this process and it should take approximately 12 hours.

In case that the survey is to be completed by employees of a company, it would be important to engage them to complete the survey during breaks such as; lunch breaks et cetera. This would provide an opportunity for more employees to participate. However in the case that external respondents is required, the survey should be conducted on a weekend when many people are not reporting to their jobs.
