I agree with your view that the approaches used by case managers also include strategy and policy approaches. This streamlines the coordination of care; enhance communication between case managers and the other stakeholders among the other ways used by case managers to improve the quality of care while ensuring efficient resource utilization (Finkelman, 2011).  I also laud your findings that hospice and palliative care institutions serve an important role in achieving this premise.

Hello Kristina, I agree with you that the strategies used by case managers apply at different levels of care. This is in line with the concept that case managers provide their services across the continuum of care. As such, these strategies apply both at the home front and during the period of hospitalization. I also find you argument that case managers should at times recommend palliative care when it is indicated, not as a way of giving up, but altering disease management depending on its progression (Summers, 2015).

Hello Christina, I agree with you that case managers can improve the quality of care offered while still achieving cost effectiveness by determining the relevance of and necessity of different diagnostic tests and treatments in liaison with attending physicians. Through their function of communicating with the patient and family members, case managers can help the patient determine when it is appropriate to seek palliative care, thereby helping stem the costs of hospitalization, while at the same time improving the quality of life for the patient (Snoddon, 2010).

Hello Mary-H, I relate to the example you have given relating to a situation where you applied evidence based practice to improve patient outcomes. I also relate to your frustration, especially at a period where you were ignorant of the concept of evidence based practice, and its importance in the nursing profession. Your optimism that the new program in your institution will succeed cannot be doubted. This is because evidenced-based practices avoids the routine practices in favor of practices that are grounded in evidence, hence the improvement in patient outcomes (Melnyk & Fineout-Overholt, 2014).

Hello Beth, your realization that the institutions in health care are changing, and so is the relevance of old practices in the nursing institution is in line with concepts from Rubenfeld & Scheffer (2010).  But by embracing evidence based practices, health institution can adopt new approaches to treatment, thereby improving the quality of care and patient outcomes. Such practices were used in your institution to explain the spike in blood sugar levels of postoperative diabetic patients, thereby serving to assuage their fears.

Hello Penelope, the fact that you were keen enough to engage with the salesperson and relate his assertions to the problems in your unit shows that you comprehended the concepts of evidence based practice. Your actions, especially where you reported back to your infections control nurse in your unit with suggestions of running trials with the Chloraprep mimics the actions of Marcia which are highlighted in Rubenfeld & Scheffer (2010). I also laud they organizational learning behavior in your institution. Even though the infections rates in your institution are below the national averages, your unit showed the desire to improve patient outcomes by reducing the rate of infection.

Hello Mary, in your initial post, you relay your ignorance of the concepts of critical thinking. However, your inquisitiveness shows the desire for knowledge, a quality of critical thinking. In this regard, I agree with your argument that you possessed the critical thinking skills even though you were deficient in the knowledge of their application. Your perseverance, especially in the face resistance in organization learning is commendable. I would recommend the used of evidence based practice and informatics to change your unit (Robert & Petersen, 2013).
