Question 1

The goal of Meaning Use can only be realize if the relevant information about the patients are properly recorded and updated in the EHR system. Thus, as a nurse I will ensure the Meaningful Use is being implemented by recording all the information I get about the patient into the system. This will ensure all the important details about the patient are not lost and they safely recorded in the system. When all the relevant information is captured by the system it will be easy to share the information. I will educate my patient and family members how to access their information (e.g. lab result, diagnostic test, medication list, allergies) online or how to download various medical forms from the EHR (AAOS, 2012). It is essential patients know how to log and out of the system without jeopardizing their privacy.
Furthermore, with all the relevant information in the EHR system, it will be easy to share the information with my colleagues and other practitioners in the hospital who are involved in the patient case and get their opinion and analysis on how best to treat the patient. This will be helpful in making informed decisions on providing care to the patient. Additionally, it will enhance collaboration with all the stakeholders in providing quality and safe health care (HealthIT.gov, 2015).
As manager it is important all the staffs in the hospital are trained on how to use EHR and how they can teach their patients to access the EHR to get information about their health. The EHR will be useful in providing evidence base care since all the information will be in the system (Nursing center 2015). It is essential to inform all the staffs the importance of Implementing Meaning Use and what the hospital and the staffs stand to gain e.g. the Medicare and Medicaid funds. Additionally, I will emphasize the importance of sharing the information with the aim of improving efficiency, safety and the quality of the health care. However, I will urge the staffs as they share information, the patient privacy should be maintained and protected. For example patients information should not be discuss in public or unauthorized person should not have access codes of the EHR system (HealthIT.gov, 2015).

Question 2

PHRs have variety of advantages such providing total privacy of patient information since the patient has total control of the information. Additionally, all health information about the patient and his/her family are contain in one place. The patient can monitor how his personal information is shared. Moreover, it provides the patient with the opportunity to access health care information anytime anywhere through a computer or Smartphone (Wolf, 2012).
Nurses can access the information in the PHR by asking the patient permission to share the needed information in the PHR. If the information in the PHR is managed by the hospital, the nurse can as for the access codes from the authorized office which deal with the PHR of the patient. Moreover, the nurse can also ask the agency which maintains the information of patient to share the information (Linda, 2008).

Some of the obstacles which may be encountered when using the PHRs

The devise which contains the information about the patient may be damaged hence the information cannot be retrieved. Additionally, patient may forget his/her password to the devise with the information hence accessing the information can be a problem (Wolf, 2012). The language used to record/store the information might be foreign to the nurse or encrypted hence understanding the information can be a problem. The patient can refuse to share the information in the PHRs due to privacy reasons. A nurse may not have the expertise to operating the devise which contain information which may hinder access to the information (Ncbi, 2006).
